    Clinical Prediction Rule for Stratifying Risk of Pulmonary Multidrug-Resistant Tuberculosis

    Multidrug-resistant tuberculosis (MDR-TB), resistance to at least isoniazid and rifampin, is a worldwide problem.To develop a clinical prediction rule to stratify risk for MDR-TB among patients with pulmonary tuberculosis.Derivation and internal validation of the rule among adult patients prospectively recruited from 37 health centers (PerĂș), either a) presenting with a positive acid-fast bacillus smear, or b) had failed therapy or had a relapse within the first 12 months.Among 964 patients, 82 had MDR-TB (prevalence, 8.5%). Variables included were MDR-TB contact within the family, previous tuberculosis, cavitary radiologic pattern, and abnormal lung exam. The area under the receiver-operating curve (AUROC) was 0.76. Selecting a cut-off score of one or greater resulted in a sensitivity of 72.6%, specificity of 62.8%, likelihood ratio (LR) positive of 1.95, and LR negative of 0.44. Similarly, selecting a cut-off score of two or greater resulted in a sensitivity of 60.8%, specificity of 87.5%, LR positive of 4.85, and LR negative of 0.45. Finally, selecting a cut-off score of three or greater resulted in a sensitivity of 45.1%, specificity of 95.3%, LR positive of 9.56, and LR negative of 0.58.A simple clinical prediction rule at presentation can stratify risk for MDR-TB. If further validated, the rule could be used for management decisions in resource-limited areas

    Forecasting Cryptocurrency Value by Sentiment Analysis: An HPC-Oriented Survey of the State-of-the-Art in the Cloud Era

    This chapter surveys the state-of-the-art in forecasting cryptocurrency value by Sentiment Analysis. Key compounding perspectives of current challenges are addressed, including blockchains, data collection, annotation, and filtering, and sentiment analysis metrics using data streams and cloud platforms. We have explored the domain based on this problem-solving metric perspective, i.e., as technical analysis, forecasting, and estimation using a standardized ledger-based technology. The envisioned tools based on forecasting are then suggested, i.e., ranking Initial Coin Offering (ICO) values for incoming cryptocurrencies, trading strategies employing the new Sentiment Analysis metrics, and risk aversion in cryptocurrencies trading through a multi-objective portfolio selection. Our perspective is rationalized on the perspective on elastic demand of computational resources for cloud infrastructures

    Social play as joint action: A framework to study the evolution of shared intentionality as an interactional achievement

    The present research was supported by the Swiss National Science Foundation (Grant No. CR31I3_166331 awarded to AB and KZ).Social play has a complex, cooperative nature that requires substantial coordination. This has led researchers to use social games to study cognitive abilities like shared intentionality, the skill and motivation to share goals and intentions with others during joint action. We expand this proposal by considering play as a joint action and examining how shared intentionality is achieved during human joint action. We describe how humans get into, conduct, and get out of joint actions together in an orderly way, thereby constructing the state of “togetherness” characteristic of shared intentionality. These processes play out as three main phases, the opening (where participants are ratified and joint commitments are established), the main body (where progress, ongoing commitments, and possible role reversals are coordinated), and the closing (where the intention to terminate the action is coordinated and where participants take leave of each other). We use this process in humans as a framework for examining how various animal species get into, maintain, and get out of play bouts. This comparative approach constitutes an alternative measure of those species’ possession of shared intentionality. Using this framework, we review the play literature on human children and different social species of mammals and birds in search of behavioral markers of shared intentionality in the coordination of play bouts. We discuss how our approach could shed light on the evolution of the special human motivation to cooperate and share psychological states with others.PostprintPeer reviewe
